IACUC Training Record Information
All personnel involved in animal work must complete an IACUC Training Record and attach it to the IACUC protocol. The Principal Investigator (PI) and any Co-Investigators (Co-Is) are required to complete a Training Record regardless of whether they will directly work with animals under this protocol.
1. Complete the IACUC online training
Online IACUC Training through SciShield
- If you are not yet registered with SciShield OR receive an error message upon login, complete the online SciShield Access Request Form.
2. EHS Occupational Health and Safety Program (OHSP)
- Complete the OHSP Online Risk Assessment Form.
- If your OHSP Online Risk Assessment indicates that medical surveillance is required, OHSP staff will send you an Animal Allergies Exposure Questionnaire, which must be completed and submitted.
- Note: Responses to the Animal Allergies Exposure Questionnaire are considered private medical information (confidential) and are sent to occupational health medical staff only—NOT to OHSP or IACUC Administrators.
