Required Training

IACUC Training Record Information

All personnel involved in animal work must complete an IACUC Training Record and attach it to the IACUC protocol. The Principal Investigator (PI) and any Co-Investigators (Co-Is) are required to complete a Training Record regardless of whether they will directly work with animals under this protocol.

All CSU Employees and Students need to complete the following:

1. Complete the IACUC online training

2. EHS Occupational Health and Safety Program (OHSP)

  1. Complete the OHSP Online Risk Assessment Form.
  2. If your OHSP Online Risk Assessment indicates that medical surveillance is required, OHSP staff will send you an Animal Allergies Exposure Questionnaire.
    • Note: Responses to the Animal Allergies Exposure Questionnaire are considered private medical information (confidential) and are sent to occupational health medical staff only—NOT to OHSP or IACUC Administrators.

3. Document and submit all training information using the IACUC Training Record Form.

Non-CSU Personnel and CSU Associates need to complete the following:

  1. Review and document all training information and waiver release form on the Non-CSU Personnel Training Record Form.
