The Principal Investigator (PI) and any Co-Investigators (Co-Is) are required to complete a Training Record regardless of whether they will directly work with animals under this protocol.<\/p>\n<\/blockquote>\n\n\n\n<h2 class=\"wp-block-heading\">All CSU Employees and Students need to complete the following:<\/h2>\n\n\n\n<h3 class=\"wp-block-heading\">1. Complete the <a href=\"https:\/\/colostate.scishield.com\/rafttraining\/course\/409\">IACUC online training<\/a><\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li>If you are not yet registered with SciShield OR receive an error message upon login, complete the online&nbsp;<a href=\"https:\/\/apps.powerapps.com\/play\/e\/default-afb58802-ff7a-4bb1-ab21-367ff2ecfc8b\/a\/20cbc9a5-576a-45cd-b409-fc4c6bbaf3c7?tenantId=afb58802-ff7a-4bb1-ab21-367ff2ecfc8b&amp;\" target=\"_blank\" rel=\"noreferrer noopener\">SciShield Access Request Form<\/a>.&nbsp;<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\">2. EHS Occupational Health and Safety Program (OHSP)<\/h3>\n\n\n\n<ol class=\"wp-block-list\">\n<li>Complete the OHSP&nbsp;<a href=\"https:\/\/wsprod.colostate.edu\/cwis86\/WOHSP\/RiskAssessment\/add\/RA.aspx\" target=\"_blank\" rel=\"noreferrer noopener\">Online Risk Assessment Form<\/a>.<\/li>\n\n\n\n<li>If your OHSP Online Risk Assessment indicates that medical surveillance is required, OHSP staff will send you an Animal Allergies Exposure Questionnaire.\n<ul class=\"wp-block-list\">\n<li>Note: Responses to the Animal Allergies Exposure Questionnaire are considered private medical information (confidential) and are sent to occupational health medical staff only\u2014NOT to OHSP or IACUC Administrators.<\/li>\n<\/ul>\n<\/li>\n<\/ol>\n\n\n\n<p class=\"wp-block-paragraph\"><a href=\"https:\/\/www.ehs.colostate.edu\/WOHSP\/Home.aspx\">CSU Environmental Health, Occupational Health and Safety Program website<\/a><\/p>\n\n\n\n<h3 class=\"wp-block-heading\">3. Document and submit all training information using the <a href=\"https:\/\/www.research.colostate.edu\/orcc\/wp-content\/uploads\/sites\/24\/2026\/03\/LastName_TrainingRecord.docx\" target=\"_blank\" rel=\"noreferrer noopener\">IACUC Training Record Form<\/a>.<\/h3>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Non-CSU Personnel and CSU Associates need to complete the following:<\/strong><\/h2>\n\n\n\n<ol class=\"wp-block-list\">\n<li>Review and document all training information and waiver release form on the <a href=\"https:\/\/www.research.colostate.edu\/orcc\/wp-content\/uploads\/sites\/24\/2026\/04\/LastName_NonCSU.docx\">Non-CSU Personnel Training Record Form<\/a>.<\/li>\n<\/ol>\n\n\n\n<h3 class=\"wp-block-heading\">Hands-on training for animal users<\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li><a href=\"mailto:lar_training@mail.colostate.edu\">Contact LAR Training Staff<\/a><\/li>\n<\/ul>\n","protected":false},"excerpt":{"rendered":"<p>IACUC Training Record Information All personnel involved in animal work must complete an IACUC Training Record and attach it to the IACUC protocol.
