1. Review CSU IACUC Training
2. Review CSU OHSP Information for Non-CSU Personnel
   
3.  Fill out a Training Record Form
   
   1. Put the date on the training form when the documents were reviewed.
   2. It is helpful if you can indicate that you are not eligible to be enrolled in CSU OHSP. If you are covered by your home institution or primary place of employment, please indicate this.
   3. Fill out the procedure training section with your animal handling experience.

Send the completed form to the IACUC Coordinator that contacted you, or send to your PI.
